Lanae Davis will be presenting at the Strengthening Colorado Families and Communities Conference in September, 2018, speaking on Pathways to Success: Developing a Model Intervention to Prevent Homelessness Among Youth Aging Out of Foster Care. Colorado’s Pathways to Success Demonstration Project is a multi-year, multi-phase project awarded through a grant to CDHS, Child Welfare Division from the Children’s Bureau to develop (Phase I), implement and test (Phase 2) a model intervention aimed at reducing homelessness among youth aging out of the foster care system. This session will describe the early implementation design including innovative ways youth voice was incorporated into the design of the model intervention, provide an overview of program implementation successes and challenges, and review approaches to identifying risk and protective factors of youth most at-risk of homelessness. In addition, details on the design of an evidence-based screening tool and the key outcome areas of permanency, well-being, education, employment and housing will be reviewed.
